How do the programs work?

How to join
To be eligible for the Healthy Weight For Life (HWFL) programs in general, you must:
- hold AIA Health Bronze Hospital cover or higher
- have a body mass index (BMI) of 28 and over
- be aged 18 and over
- have not previously participated in a Healthy Weight For Life program funded by AIA Health
- be willing to provide your progress data to the Healthy Weight For Life team through the program (including weight, waist circumference, joint symptom scores)
- have GP or medical practitioner approval

Complete the assessment form via the link below to help the HWFL team identify which program would be the most suitable for you.
Once you’ve completed and submitted your assessment form, a HWFL team member will be in touch to discuss your situation further and help you enrol in the program that is most suited for you.
Please note, if determined necessary by the team member, you may have to obtain approval from your GP or specialist to enrol in the programs. If this is the case, you will need ask your doctor to fill out the GP Approval Form (which HWFL will provide). You can then return your completed form to Prima Health Solutions via email to hwfl@hwfl.com.au or fax to (02) 9938 5090.
Frequently asked questions
- hold AIA Health Bronze Hospital cover or higher
- have a body mass index (BMI) of 28 and over
- be aged 18 and over
- be treated for or have a history of at least one of the following: high blood pressure, high cholesterol, angioplasty or stent, heart attack or stroke, coronary bypass surgery, cardiac arrhythmia/angina, or obstructive sleep apnoea
- have not previously participated in a Healthy Weight For Life program funded by AIA Health
- be willing to provide your progress data to the Healthy Weight For Life team through the program (including weight, waist circumference, joint symptom scores)
- have GP or medical practitioner approval
- hold AIA Health Bronze Hospital cover or higher
- have a BMI of 28 and over
- be aged 18 and over
- be diagnosed with knee or hip osteoarthritis and considering joint replacement surgery
- have not previously participated in a Healthy Weight For Life program funded by AIA Health
- be willing to provide your progress data to the Healthy Weight for Life team throughout the program (including weight, waist circumference, joint symptom scores)
- have GP or medical practitioner approval
- hold AIA Health Bronze Hospital cover or higher
- have a BMI of 28 and over
- be aged 18 and over
- have type 2 diabetes
- have not previously participated in a Healthy Weight For Life program funded by AIA Health
- be willing to provide your progress data to the Healthy Weight For Life team throughout the program (including weight, waist circumference, joint symptom scores)
- have GP or medical practitioner approval
- Decrease in waist circumference
- Less joint or muscle pain
- Decrease in blood pressure
- Improved blood glucose levels
- Improved cholesterol and triglycerides
- Increased energy
- Improvements in personal body image and sense of wellbeing
- Decreased snoring
