This offer is made available exclusively to AIA Australia Limited (‘AIAA’) Priority Protection life insurance customers, who hold a registered and activated AIA Vitality membership and have received a promotional offer email communication directly from AIAA.  

Submitting your information in the offer registration form does not constitute acceptance of your registration. This offer is subject to Unified Healthcare Group (‘UHG’) registered nurse and AIA Vitality Health Check at home (‘Health Check’) kit availability. It may take up to eight (8) weeks from the time of registration to the time of an available appointment.

By clicking submit in the Health Check digital registration form, you consent to having the personal information you entered into the form shared with representatives of UHG, for the sole purpose of providing you with the Health Check service.

Forming part of the Health Check service, UHG will notify AIAA upon successful completion of each Health Check. This notification will include information limited to a participant’s full name and AIA Vitality membership number.
